Dental office automations: five front-desk problems, five tested fixes
Dental practice automation for small US offices on Open Dental: missed-call text-back, unscheduled treatment follow-up, overdue recall texts, review requests with a privacy check on replies, and insurance eligibility checks three days before each visit. Each recipe is an n8n workflow with setup steps, a monthly cost from the vendors' own price pages and the result of our test run. No AI model is used: every rule is written out, texts carry no names, treatment or fees, and a patient marked as not OK to text in Open Dental never gets one.

Why these five
Calls and chairs: missed calls and recall
Peerlogic's analysis of 4,280 calls to 26 practices (as summarised by a secondary source): 38% of calls went unanswered, and only 42% of new-patient calls ended in a booking. Dental missed-call statistics (summary of Peerlogic's study of 4,280 calls at 26 practices, and Patient Prism figures) ADA Health Policy Institute, Q3 2025: 90.8% of practices trying to hire a dental hygienist said recruiting one was challenging. Treatment that never gets booked
Henry Schein One's benchmark: average providers had a case acceptance rate of 46%, against 83% for the top performers. Henry Schein One: average providers are losing hundreds of thousands in unscheduled treatment
Reviews and insurance
HHS Office for Civil Rights, Dec 2022: New Vision Dental paid $23,000 and took on a two-year corrective action plan after replies to Yelp reviews disclosed patients' names, treatment and insurance details. HHS press release: OCR settlement with New Vision Dental over PHI in replies to online reviews (Dec 14, 2022) CAQH Index figures, as reported in a Colorado Dental Association news piece written by a vendor: about 12 minutes per manual eligibility check, around 24 with follow-up. What each recipe does
Missed calls: a text-back with no patient details
When a call to the practice is missed, the caller gets one short text within seconds: sorry we missed you, we'll call back, here is the online booking link (after hours: we're closed, here is the emergency line). The text never contains a name, appointment or treatment. Front-desk staff get a callback row in a Google Sheet, the call is logged on the patient's chart in Open Dental, and STOP replies are written back to Open Dental so no recipe texts that number again. Setup, costs and test results.
Unscheduled treatment follow-up from Open Dental
Every week the workflow finds treatment-planned procedures in Open Dental that have no future appointment and builds a call list for the treatment coordinator: patient, planned codes, fee and date presented. Patients who agreed to texts get at most two generic nudges per plan, 30+ days apart ('you have a follow-up visit to schedule with us'), never the procedure, tooth or fee. Patients who book drop off; patients who reply or opt out become call-only. Setup, costs and test results.
Overdue recall and reactivation texts
Each weekday morning the workflow reads Open Dental's recall list for patients who are past due and not booked, sends up to three generic reminders 14+ days apart ('it's time to book a checkup', with your online booking link) and records each one on the recall the same way Open Dental's own recall list does. Families sharing a phone get one text. Patients without text consent, and those who got three reminders and still didn't book, go on a call list. Setup, costs and test results.
Review requests and HIPAA-safe review replies
Part 1: every evening, each patient with a completed visit and text consent gets the same short request for a Google review, at most once a year and once per family phone. There is no 'were you happy?' pre-screen and no incentive. Part 2: when a new public review arrives, the office manager gets an email with a privacy-safe reply template and, if a staff draft was attached, a list of phrases in it that would confirm the reviewer is a patient or reveal care, dates or amounts. Nothing is posted automatically and the reviewer is never looked up in Open Dental. Setup, costs and test results.
Insurance verification prep, 72 hours out
Each weekday at 7 am the workflow takes every appointment three days out, follows the patient's insurance in Open Dental (plan, subscriber, carrier), runs a real-time eligibility check through Stedi and writes a checklist for the front desk: active or not, deductible and annual maximum left, coinsurance, and what to do next. Active coverage is marked verified in Open Dental with a note. Inactive plans, payer errors, low remaining maximums and patients without insurance are flagged for a person. Patients get no text. Setup, costs and test results.
Overlap to know about: Weave has its own missed-call text, and Open Dental has Web Sched Recall, eReminders, eConfirmations and a Treatment Finder report. If you already use those, you may not need our version, and you should not run both. We dropped a no-show recipe for that reason: Open Dental's eConfirmations already cover it.
HIPAA, TCPA and BAAs: the rules we design around
Not legal advice, and not a compliance guarantee. These recipes are built to keep risk low, but nobody can make your practice compliant with a download. Check with your compliance officer or counsel and your state dental board before you turn any of them on.
- No patient details in texts. Every text is generic: practice name, a booking link or phone number, "Reply STOP to opt out". No names, procedures, teeth, fees or balances. HHS allows reminders as part of treatment (FAQ 286) and electronic messages with reasonable safeguards such as limiting what is sent (FAQ 570); the minimum necessary standard is in 45 CFR 164.502(b).
- Consent comes from Open Dental. Scheduled texts (treatment, recall, review requests) go only to patients whose record says OK to text. The missed-call reply goes to whoever just called, unless that number is marked not OK to text. STOP and plain-English opt-outs are written back to the patient record, so every recipe and Open Dental's own messages see them. Patients' requests for other ways of contact are covered by 45 CFR 164.522(b).
- TCPA limits. The FCC's exemption for healthcare texts in 47 CFR 64.1200(a)(9)(iv) has conditions: 160 characters or less, one message a day and three a week per patient, no billing or financial content, and an easy opt-out honored immediately. All five recipes share one ledger that enforces 1 text per 24 hours and 3 per 7 days per phone number, and no text includes a balance or payment link. Whether reactivation, unscheduled-treatment or review texts fit that exemption is a question for counsel.
- Business associate agreements. A vendor that stores or processes patient information for you needs a BAA (HHS on cloud services). n8n does not offer one for n8n Cloud (n8n staff, Jan 2026), so self-host n8n on infrastructure covered by a BAA. Twilio's HIPAA offering needs Security or Enterprise Edition and a signed addendum, priced by Twilio Sales (Twilio docs). Staff lists live in Google Sheets only after your Workspace super admin accepts Google's BAA (Google help). Insurance checks run through a Stedi HIPAA account, which is what Stedi's BAA covers (Stedi BAA).
- Review replies. HHS settled with New Vision Dental for $23,000 and a two-year corrective action plan after replies to Yelp reviews disclosed patient information (HHS, Dec 2022). Our template never confirms the reviewer is a patient, and nothing is posted automatically. Review requests go to every eligible patient the same way, because Google's policy bars selective asking and incentives. Advertising must not be false or misleading (ADA Code, section 5.F), and your state board may have its own rules.
US only for now. Dentrix, Eaglesoft, Curve and NexHealth versions are not built, and we make no claim that these recipes work with them. Several pain figures come from vendors that sell dental software; each is labeled on the recipe page. All recipes were verified with mocks; no real patient data was used.

TCPA, A2P 10DLC and CASL for HVAC and plumbing shops that text back missed callers: what the rules say, where it's gray, and a setup checklist.
How we test
Every workflow was imported into n8n 2.45.4 and run end to end on our test machine. Open Dental's API, Twilio, Stedi, Google Sheets and the email sender (Resend) were replaced by a local mock server built from their official API docs: it checks the same request shapes and required fields, enforces Open Dental's request throttle (HTTP 429) and records every call. We also compared the mock's field names with Open Dental's public test database (read-only GET requests with the test key Open Dental publishes, no writes). These recipes use no AI model; every decision is a written rule. Patients, plans and phone numbers are invented. Verified with mocks; no real patient data used, and nothing was sent to a real phone, patient, payer or account.
Software covered: Open Dental (Dentrix, Eaglesoft and NexHealth not built yet). Prices checked October 2026.
