Dental missed call text back: what it costs to build vs. buy in 2026
A line-by-line cost breakdown of a dental missed-call text back: Twilio, A2P 10DLC, the Open Dental API and n8n, next to what an all-in-one phone platform charges.
A do-it-yourself dental missed call text back runs about $34 to $43 a month for one location that misses around 120 calls a month, plus roughly $20 to $61 in one-time texting registration fees, plus the server you run n8n on. Most of that is the Open Dental API ($30 a month per location), not the texts, which cost under a cent each. If your office already pays for a phone platform that includes missed-call texts, such as Weave (listed "starting from $199 per month"), turn that feature on first and skip the build.
All the arithmetic below is illustrative, built from each vendor's published prices as checked on October 28, 2026. It is not a measured result from a real practice.
Not legal or compliance advice. We build and test automations; we're not lawyers or HIPAA consultants. Before any automated text goes to a patient, have your compliance officer approve the wording and where the data lives, and check your state dental board's rules. Our dental texting checklist covers HIPAA, BAAs and the TCPA in detail.
What a missed call text back does in a dental office
When a call to the front desk rings out, goes busy or fails, the caller gets one short text within a minute or so: sorry we missed you, we'll call back, here's the online booking link. After hours, the text says the office is closed and gives the emergency number. The front desk gets a callback list in the morning.
The dental version has two extra jobs that a plumber's version doesn't:
- Say nothing about the patient. The same cell phone is often on several family charts, and the person holding it may not be the patient. The text should never include a name, a procedure or a balance. For email, HHS says unencrypted messages to patients are allowed with "reasonable safeguards", such as "limiting the amount or type of information disclosed" (HHS FAQ 570, federal agency guidance). Our recipe applies the same idea to texts: a generic message with nothing about the patient in it.
- Respect "no texts" flags. If a patient has asked not to be texted, the automation has to check the chart before it sends, and a STOP reply has to switch texting off on every chart with that number.
Before you pay for anything: the cheaper fixes
Work through these first. Each one costs less than a build.
- Check what you already pay for. Phone platforms built for dental offices often include missed-call texts. Weave's missed call text page (vendor page) describes separate messages for open and closed hours, sent from the office number. If you have it, configure it.
- Fix the phone tree. A rollover line, a second ring group or a voicemail greeting with your online booking link costs nothing.
- Staff the lunch hour. If the callback list shows most misses cluster at 12:00 to 1:00, a staggered lunch may solve more than software.
If none of that fits, here is what the do-it-yourself route costs.
The cost breakdown, line by line
Prices below are each vendor's own published list prices, checked October 28, 2026. Taxes and any volume discounts are not included.
| Line item | What it's for | Price | Source |
|---|---|---|---|
| Twilio local number | The number your office line routes through | $1.15 / month | Twilio US SMS pricing (vendor) |
| Twilio SMS | Each text sent or received, per 160-character segment | $0.0083 per segment | Twilio US SMS pricing (vendor) |
| Carrier fees | Added per outbound segment by the mobile carrier | AT&T $0.0035, T-Mobile $0.0045, Verizon $0.005, others $0.0042 | Twilio US SMS pricing (vendor) |
| A2P 10DLC brand | One-time carrier registration of your business | $4.50 one-time; Standard brands add $41.50 vetting | Twilio 10DLC brand registration docs (vendor) |
| A2P 10DLC campaign | One-time vetting plus a monthly fee for the texting "campaign" | $15 one-time; $1.50 / month (low-volume mixed) to $10 / month (standard) | Reseller page, see note below |
| Open Dental API | Reading patients by phone, writing commlogs, setting "no texts" on STOP | $30 / month per location (the tier that includes patient updates) | Open Dental API permissions (vendor) |
| n8n | Runs the workflow | Community Edition: free to self-host (you pay for the server) | n8n pricing (vendor) |
| Google Sheets | Callback list and reply log | Included in a paid Google Workspace plan you likely already have | Your existing plan |
A note on the 10DLC campaign fees. Twilio's own fee article loads only in a browser with JavaScript, and we couldn't read it this session. The $15 vetting fee and the $1.50 to $10 monthly campaign fees above come from Salesmate's A2P 10DLC fee page (a CRM vendor that resells Twilio messaging). Its brand fee is out of date, so treat its numbers as a guide and confirm the current figures in your Twilio console before you register. A2P 10DLC itself is required: Twilio's A2P 10DLC overview (vendor docs) says anyone sending texts from a local number to US phones "must register".
Why the Open Dental line is $30, not free. Open Dental's free API tier is read-only. The text-back has to write to the chart: a commlog for each text and "TxtMsgOk = No" when someone replies STOP. On Open Dental's permissions page, the $15 tier covers commlogs but not patient updates; the $30 tier covers "all except Payments, PayPlans and Special". Open Dental's page also says the fees are paid by the developer, so if a consultant holds your API key, ask who pays.
Illustrative monthly total for one location
Illustrative arithmetic, not a measured result. Assume 120 missed calls a month from 100 different numbers, and about 30 replies.
| Item | Math | Monthly |
|---|---|---|
| Texts out and replies in | 130 segments x $0.0083 | $1.08 |
| Carrier fees on outbound texts | 100 x $0.0035 to $0.005 | $0.35 to $0.50 |
| Twilio number | $1.15 | |
| 10DLC campaign fee | low-volume mixed to standard | $1.50 to $10.00 |
| Open Dental API | one location | $30.00 |
| Total | about $34 to $43 |
One-time fees in the first month: $4.50 brand plus $15 campaign vetting ($19.50), or $61 if you register as a Standard brand. Add your n8n server. If your compliance officer decides any message could contain patient information, Twilio requires a HIPAA account, which needs "Twilio Security Edition or Enterprise Edition" plus a signed Business Associate Addendum (Twilio HIPAA accounts docs, vendor). Those editions are sold through Twilio's sales team with no public price, so get a quote before you plan on it. The text in our recipe is generic so that it carries no patient details, but that decision is your compliance officer's, not ours.
Build vs. buy: which one fits your office
| Do-it-yourself (n8n + Twilio + Open Dental API) | All-in-one phone platform | |
|---|---|---|
| Published price | About $34 to $43 / month in this example, plus a server | Weave lists "starting from $199 per month" (Weave pricing, vendor) |
| What you get | Only the text-back, callback list and opt-out sync | Phones, texting, reminders, reviews and more in one bill |
| Who maintains it | You or your IT person | The vendor |
| Best for | Offices on Open Dental that already have someone comfortable with n8n | Offices that want one vendor, or already pay for one |
The do-it-yourself route is cheaper on paper, but it only pays off if someone on your team will keep it running. If nobody will, the platform is the better buy even at a higher price.
What we tested, and what we didn't
Our dental missed call text back recipe was imported into n8n and run end to end against a local mock server, not live systems. The mock stands in for Twilio, the Open Dental API and Google Sheets, checks request shapes and replays Open Dental's rate-limit (HTTP 429) responses. No real patient, phone number or account was used, nothing was sent to a real phone, and the workflow uses no AI model: every decision is a written rule.
The latest run (October 28, 2026) passed 10 of 10 scenarios:
- T1: a new caller during office hours gets one generic text with the booking link, and a callback row is added. No chart is touched.
- T2: a family number after hours (one cell on three charts) gets the "we're closed" text with the emergency line and no names; the commlog goes on the guarantor's chart.
- T3: the same caller an hour later gets no second text (one text per number per 12 hours).
- T4: a patient whose chart says no texts gets no text; the callback row says "call only".
- T5: an answered call triggers nothing.
- T6: a STOP reply sets "no texts" on all three family charts with that number.
- T7: a plain-English "please stop texting me" is treated as an opt-out, with one confirmation text.
- T8: an ordinary reply is stored in the replies sheet, and the staff alert text carries no message content.
- T9: a forged webhook (bad Twilio signature) is ignored.
- T10: an Open Dental rate-limit reply is retried after the wait, and the run finishes.
The text the tests produced during office hours (with an invented practice name and link):
Maple Grove Dental: sorry we missed your call. We'll call you back shortly, or book online: https://example.com/maple-grove/book Reply STOP to opt out.
That's one 160-character segment, so it bills as one text. What we did not test: real carrier delivery, 10DLC approval, your phone system's routing, or how patients respond. The STOP handling follows the opt-out words listed in the federal TCPA rule (47 CFR 64.1200(a)(10), federal regulation), and our post on whether missed-call text back is legal explains the rest.
